Are anchovies bad for gout?
Anchovies sit near the top of the purine charts, and high-purine seafood is linked to more gout flares. Where that risk is real, where diet ranks against medication, and how to find your own line.
The little fish that punches above its weight
Gout starts with uric acid, the waste your body makes when it breaks down purines. Purines are building blocks of DNA and other molecules, so any cell-dense food carries them. When uric acid runs high, it can crystallize in a joint, and those needle-sharp crystals are what set off the heat and pain of an attack. Anchovies are packed with purines because you eat the whole small, cell-rich fish, organs and all, in a concentrated salty bite.
Think of purines as coins you have to cash in. Most foods hand your kidneys a few coins at a time. Anchovies hand over a fistful at once, and a backed-up till is how uric acid climbs. That puts them in the same top tier as sardines, herring, mackerel roe and organ meats on every purine chart, well above chicken, most white fish, or a steak.
The flare link is established at the population level. In a 12-year Harvard study of 47,150 men with no history of gout, those eating the most seafood had a 51 percent higher rate of new gout than those eating the least (relative risk 1.51). That is an association measured across a crowd. Anchovies sit at the high-purine end of the seafood it points to, though no single study has pinned the risk on anchovies alone.

Your trigger isn't the chart's average
Purine charts rank foods by content, not by what happens in your joints. Two people can eat the same Caesar salad and only one wakes up with a flaring toe, because flare risk rides on your baseline uric acid, your kidneys, your medication, alcohol, dehydration and what else shared the plate. The chart says anchovies are high. It cannot say whether they are high for you, tonight.
The only way to learn your own line is to watch this food against how you actually feel over the days after. Log the anchovies, the beer beside them, the heat or stiffness that shows up a day or two later, and a pattern you could never hold in your head starts to surface. - Treat anchovies as an occasional food rather than a staple if you get flares. A few fillets on a pizza now and then beats a regular anchovy habit.

How many anchovies are safe with gout?
There is no universal safe number. It depends on your uric acid, medication and overall diet. As a rule of thumb, the occasional few fillets are lower-stakes than eating them regularly. If you flare often, treat anchovies as a sometimes food and track how your joints respond.
Are anchovies worse than sardines or other oily fish for gout?
They are in the same top purine tier as sardines, herring and mackerel, so all are commonly grouped as 'limit' fish for gout. Salmon and most white fish are lower. The oily-fish omega-3s are good for general health, which is why the usual advice is to limit the highest-purine fish rather than drop fish entirely.
Will cutting out anchovies fix my gout?
Unlikely on its own. The 2020 American College of Rheumatology guideline only conditionally recommends limiting purines, and notes the evidence that diet meaningfully lowers uric acid is limited. Urate-lowering medication is the proven lever. Diet is a helpful add-on, not a replacement for treatment, so keep taking what your doctor prescribed.
What about Worcestershire sauce or Caesar dressing that contain anchovies?
The amount of anchovy in a splash of Worcestershire or a Caesar dressing is tiny next to a serving of whole fillets, so the purine load is much smaller. A whole-anchovy pizza topping or anchovy pasta is the bigger contributor to watch.
